Craig Ventresco and Meredith Axelrod are playing live in their living room daily while the coronavirus pandemic runs. Sometimes it's just Craig, sometimes it's both of them. He's probably best known to most from his work with the band Bo Grumpus and his contributions to the soundtrack of "Ghost World". Give him a listen and drop him some cash if you've got it. That cover plate is certainly unique - I don’t recall seeing one like that before. The nut appears to be scalloped. The back seam appears to be separating.
I’ll try sharing this link to see if anyone else has more information or ideas.
